This case report details a rare instance of acute lymphoblastic leukaemia (ALL) presenting initially as a cervical mass with obstructive uropathy. The diagnosis of precursor B cell ALL was incidental, highlighting a unique clinical presentation.

Background:

  • Acute lymphoblastic leukaemia (ALL) rarely involves the cervix.
  • Cervical masses are typically neoplastic or inflammatory, rarely presenting as a leukaemic infiltration.

Observation:

  • A young woman presented with abdominal pain and vaginal bleeding, found to have a large cervical mass and obstructive uropathy.
  • Incidental diagnosis of CALLA-positive precursor B cell ALL was made via peripheral blood flow cytometry.
  • Histopathology and immunohistochemistry confirmed B cell ALL involvement in the cervical mass.

Findings:

  • This case represents an extremely rare initial presentation of precursor B cell ALL.
  • The cervical leukaemic infiltration mimicked advanced cervical malignancy.
  • The patient's history lacked typical leukaemic signs and symptoms.

Implications:

  • Highlights the importance of considering leukaemic infiltration in cervical masses, even without overt leukaemic symptoms.
  • Underscores the diagnostic challenge when leukaemic involvement presents as a primary cervical neoplasm.
  • Adds to the sparse literature on cervical leukaemic infiltration, particularly in young women.
