Summary

Postictal psychiatric symptoms (PPS) like anger and irritability are common after seizures. Increased EEG slowing post-seizure is linked to these symptoms, suggesting a biological mechanism in epilepsy.

Background:

  • Postictal psychiatric symptoms (PPS) are frequent but understudied in epilepsy.
  • The link between seizures and worsening psychiatric symptoms is not well understood.

Purpose of the Study:

  • Prospectively identify PPS during and after epilepsy monitoring unit (EMU) admission.
  • Characterize postictal physiological changes associated with PPS.

Main Methods:

  • Prospective enrollment of patients admitted to the EMU.
  • Administered psychometric questionnaires to assess anger, anxiety, depression, and paranoia.
  • Measured postictal EEG slowing using Hilbert transform and quantified seizure frequency.

Main Results:

  • 8 out of 33 participants experienced significant PPS within three days post-seizure.
  • Anger/hostility was the most common PPS.
  • PPS+ group showed greater postictal EEG slowing and experienced more seizures compared to the PPS- group.

Conclusions:

  • PPS, particularly anger and irritability, may be more prevalent than previously thought.
  • Quantitative EEG provides physiological evidence for a biological mechanism underlying PPS.
  • This study enhances understanding of postictal neurophysiology in epilepsy.
