Summary
Infective endocarditis with internal iliac artery aneurysm is rare. This case shows that antibiotic therapy alone can lead to aneurysm regression, detectable with PET/CT.

- The aneurysm resolved with antibiotic therapy alone, without the need for surgical intervention on the vessel.
Implications:
- Internal iliac artery aneurysms associated with IE may regress with antibiotic treatment.
- PET/CT is a valuable tool for diagnosing and tracking these rare vascular complications.
