Summary

This case study highlights a rare instance of severe mitral regurgitation caused by a posterior mitral leaflet cleft in a patient with hypertrophic cardiomyopathy. Surgical cleft correction successfully resolved the mitral regurgitation, offering a new therapeutic approach.
