Background:

  • Status epilepticus (SE) is a critical neurological emergency with significant mortality.
  • Accurate and timely identification of SE risk in emergency departments (EDs) is crucial for patient management.

Purpose of the Study:

  • To validate the novel ADAN (Abnormal speech, ocular Deviation, Automatisms, and Number of motor epileptic seizures) scale for predicting high risk of SE.
  • To assess the diagnostic performance of the ADAN scale in an emergency department setting.

Main Methods:

  • A prospective, multicenter, observational study involving 527 adults with suspected epileptic seizures across 4 hospital EDs.
  • Consecutive patient recruitment over a 27-month period.
  • Analysis of clinical characteristics and ADAN scale criteria upon ED arrival to determine SE prediction.

Main Results:

  • The ADAN scale demonstrated a high predictive power of 90.9% for a final SE diagnosis.
  • All 4 ADAN criteria were independently associated with SE diagnosis (P < .001).
  • The scale effectively stratified patients into low (8.7%), moderate (46.6%), and high (92.6%) risk groups for SE.

Conclusions:

  • The ADAN scale is a validated, straightforward clinical instrument for identifying patients at high risk for SE in the ED.
  • This tool can assist emergency physicians in rapid SE risk assessment.
  • Prospectively validated ADAN scale improves diagnostic accuracy for SE.
