    Summary

    Prophylactic metronidazole significantly reduced postabortal infection rates in first-trimester abortions. This antibiotic therapy is effective in preventing genital infections following the procedure.

    Background:

    • Postabortal infections are a significant complication of first-trimester abortions.

    Purpose of the Study:

    • To evaluate the efficacy of prophylactic metronidazole in preventing postabortal genital infections.
    • To assess the impact of metronidazole prophylaxis on women with a history of pelvic inflammatory disease.

    Main Methods:

    • A double-blind, placebo-controlled trial involving 119 women undergoing elective first-trimester abortions.
    • Participants received either metronidazole (400 mg doses) or a placebo around the time of the procedure.
    • Infection rates and antibiotic prescriptions were compared between groups.

    Main Results:

    • Metronidazole prophylaxis significantly reduced postabortal genital infection rates (3.9% vs. 20.4% in placebo group, P < .025).
    • Women with a history of pelvic inflammatory disease had a higher risk of infection.
    • Prophylaxis did not significantly reduce hospital days or influence infection frequency in women with prior pelvic inflammatory disease.

    Conclusions:

    • Prophylactic metronidazole is effective in reducing the incidence of postabortal genital infections after first-trimester abortions.
    • Metronidazole prophylaxis may be a valuable strategy for infection prevention in this patient population.
