Background:

High-quality, sustainable implementation of hypertension (HTN) evidence-based practices (EBPs) is vital for equitable care.
Implementation challenges for HTN management span multiple contextual levels, extending beyond individual patients.
Limited research has systematically engaged community and healthcare perspectives to inform HTN intervention trial design.

Purpose of the Study:

To synthesize community and healthcare informants' perspectives on contextual factors influencing hypertension management.
To gather recommendations for tailoring evidence-based practices for high-quality, sustainable implementation in a multiethnic safety-net population.

Main Methods:

Conducted structured discussions with race/ethnicity-specific community action boards (CABs).
Performed semistructured qualitative interviews with 41 healthcare system personnel.
Utilized rapid content analysis to summarize findings and EBP tailoring recommendations.

Main Results:

Identified seven key themes of contextual considerations: social determinants, healthcare engagement, clinical interaction, system operations, standardization, patient education, and partnerships/funding.
Generated nine themes for tailoring recommendations, including addressing complex contexts, social needs, system operations, linguistic/cultural adaptation, and community partnerships.

Conclusions:

Engaging community and healthcare informants grounds implementation strategies in the multifaceted contexts of diverse patients.
Reframing inequitable implementation as a multilevel social issue, rather than an individual one, is essential for effective interventions.
This approach supports the development of sustainable and equitable hypertension management programs within safety-net settings.
