Context:

Chronic calcifying pancreatitis frequently leads to obstructive calculi in the main pancreatic duct.
These calculi can cause significant pain and complications, necessitating effective removal strategies.

Purpose:

To assess the efficacy and safety of electrohydraulic lithotripsy (EHL) for removing main pancreatic duct stones.
To evaluate the utility of the ultrathin SpyGlass DS endoscope in this procedure.

Summary:

A study of 29 patients with chronic calcifying pancreatitis and main pancreatic duct stones was conducted between 2018 and 2023.
Complete removal of calculi (≥5 mm) in a single procedure was achieved in 86% of patients.
The digital SpyGlass DS system facilitated pancreatoscopy with EHL.

Impact:

Pancreatoscopy with EHL using the SpyGlass DS system is demonstrated as a highly effective method for treating main pancreatic duct calculi.
This technique offers a minimally invasive and successful therapeutic option for patients with obstructive pancreatic stones.
