[Prophylactic cerclage in placenta praevia?].

R Berg, A Eisinger

    Zentralblatt Fur Gynakologie
    |January 1, 1985

    Summary

Prophylactic cerclage for diagnosed placenta previa before 30 weeks is not recommended. Placental migration means abnormal insertion is only confirmed later in pregnancy.

    Area of Science:

    • Obstetrics and Gynecology
    • Maternal-Fetal Medicine
    • Diagnostic Ultrasound

    Context:

    • Placenta previa diagnosis often occurs in the second trimester.
    • Early diagnosis can lead to interventions like prophylactic cerclage.
    • The phenomenon of placental migration is well-documented.

    Purpose:

    • To evaluate the efficacy of prophylactic cerclage in cases of early diagnosed placenta previa.
    • To determine the optimal timing for interventions related to placenta previa.

    Summary:

    • Sonographic diagnosis of placenta previa occurred in 12 patients between 17-24 weeks gestation.
    • Prophylactic cerclage was performed in all patients between 18-24 weeks.
    • Subsequent ultrasound controls did not confirm placenta previa in any case, suggesting early diagnoses may resolve due to placental migration.

    Impact:

    • Findings suggest prophylactic cerclage for placenta previa diagnosed before 30 weeks is not indicated.
    • Interventions for placenta previa should be considered only after 30 weeks gestation.
    • This study impacts clinical guidelines regarding the management of early-onset placenta previa.
