Parapsoriasis with hyper-IgE.

• A 56-year-old man presented with generalized chronic dermatitis.
• Diagnosis revealed parapsoriasis en plaque with significantly elevated IgE levels (12,000-22,000 ng/ml).
• Lymphocyte cultures demonstrated a diminished response to mitogens.

Findings:

• Direct immunofluorescence of affected skin revealed IgE bound to epidermal and dermal mast cells.
• The patient exhibited characteristics of parapsoriasis en plaque associated with hyper-IgE.

Implications:

• This case suggests parapsoriasis with hyper-IgE may be a manifestation of 'hypersensitive immunosurveillance'.
• This immune state could potentially help patients reject malignant cells.
• It may retard the progression from benign parapsoriasis to malignant mycosis fungoides.
