    Summary

Propranolol hydrochloride, a beta-blocker, can trigger generalized pustular psoriasis in patients with pre-existing plaque psoriasis. Discontinuation of the drug and methotrexate treatment led to symptom resolution.

    Area of Science:

    • Dermatology
    • Pharmacology

    Background:

    • Psoriasis is a chronic inflammatory skin condition.
    • Generalized pustular psoriasis is a rare and severe form of psoriasis.

    Observation:

    • An 80-year-old male with a 40-year history of plaque psoriasis developed generalized pustular psoriasis.
    • The onset of pustular psoriasis occurred three days after starting propranolol hydrochloride therapy.
    • Recurrence of symptoms was observed upon re-challenge with propranolol.

    Findings:

    • Propranolol hydrochloride, a beta-blocker, is identified as a potential causal agent for generalized pustular psoriasis.
    • Methotrexate sodium administration and propranolol discontinuation led to the resolution of skin lesions and systemic symptoms.

    Implications:

    • This case highlights the importance of considering beta-blockers as potential triggers for pustular psoriasis.
    • Further research is warranted to understand the mechanisms linking propranolol to pustular psoriasis.
