Summary

This study on hypoglossal nerve stimulator (HNS) uptitration found that 60% of patients reached therapeutic amplitude within 4 months. Amplitude increased monthly, but predictors for achieving optimal HNS therapy require more research.

Area of Science:

  • Sleep Medicine
  • Biomedical Engineering

Background:

  • Obstructive sleep apnea (OSA) is a common disorder impacting millions.
  • Hypoglossal nerve stimulation (HNS) offers an alternative treatment for moderate to severe OSA.
  • Understanding HNS therapy titration is crucial for optimizing patient outcomes.

Purpose of the Study:

  • To investigate changes in hypoglossal nerve stimulator (HNS) amplitude and usage patterns during the initial uptitration phase.
  • To determine the time course for patients achieving their therapeutic HNS amplitude.
  • To explore factors influencing HNS therapy effectiveness.

Main Methods:

  • Prospective study analyzing HNS therapy parameters (amplitude, duration, pause times) over the first 4 months post-implantation.
  • Statistical analysis to track monthly amplitude changes and identify patients reaching therapeutic levels.
  • Comparison of patient characteristics (BMI, AHI, RDI) between groups based on achieving therapeutic amplitude.

Main Results:

  • Average HNS therapy amplitude significantly increased monthly from baseline (0.7 V) to month 4 (1.8 V) (P < .001).
  • By 4 months, 60% of patients achieved their therapeutic HNS amplitude.
  • Patients not reaching therapeutic amplitude by month 4 had a higher average therapeutic amplitude (2.6 V) compared to those who did (1.6 V) (P < .05).

Conclusions:

  • HNS therapy amplitude increases substantially during the initial 4 months of uptitration.
  • A significant portion of patients reach therapeutic amplitude within this period, but individual variability exists.
  • Further research is needed to identify predictors for achieving therapeutic HNS amplitude and optimize usage patterns.
