Summary

T1-weighted DANTE-SPACE MRI is a reliable method for assessing carotid artery walls in patients with stenosis. This imaging technique demonstrates high reproducibility for evaluating vessel wall characteristics in stroke patients.

Background:

  • T1-weighted (T1W) imaging using delay alternating with nutation for excitation-sampling perfection with application-optimized contrasts using different flip angle evolution (DANTE-SPACE) is a preferred MRI technique for vessel wall evaluation.
  • Carotid artery stenosis evaluation is crucial for stroke risk assessment.

Purpose of the Study:

  • To assess the intra- and inter-rater reproducibility of carotid wall segmentation using T1W DANTE-SPACE MRI.
  • To evaluate the reliability of this technique in patients with symptomatic internal carotid artery (ICA) stenosis.

Main Methods:

  • Prospective study of 25 patients with acute stroke/TIA and 50-99% ICA stenosis.
  • High-resolution 3.0-T carotid MRI using T1W DANTE-SPACE sequence.
  • Manual segmentation of vessel wall and lumen by two independent radiologists; analysis using ICC, DSC, and HD.

Main Results:

  • High intra-rater reproducibility for carotid wall volume (ICC=0.965), inner lumen volume (ICC=0.990), and normalized wall index (ICC=0.962).
  • Good inter-rater reproducibility for carotid wall volume (ICC=0.856), inner lumen (ICC=0.981), and normalized wall index (ICC=0.904).
  • Excellent agreement for symptomatic stenosis evaluation (perfect ICCs).

Conclusions:

  • T1W DANTE-SPACE MRI provides reproducible carotid wall evaluation in patients with symptomatic ICA stenosis.
  • The technique is reliable for assessing vessel wall characteristics relevant to stroke risk.
