Hospitals that previously participated in clinical studies showed better adherence to acute uncomplicated diverticulitis (AUD) management guidelines. This suggests clinical trial experience improves guideline implementation for AUD.
Area of Science:
- Gastroenterology
- Clinical Research
- Healthcare Management
Background:
- Acute uncomplicated diverticulitis (AUD) management guidelines exist.
- Variability in clinical practice for AUD is common.
- The impact of prior clinical study participation on guideline adherence is not well understood.
Purpose of the Study:
- To determine if previous involvement in clinical trials influences adherence to AUD management guidelines.
- To compare AUD management practices between hospitals with and without prior clinical trial experience.
Main Methods:
- Retrospective cohort study of 449 patients with AUD across six hospitals.
- Comparison of antibiotic use and inpatient vs. outpatient management between hospitals that participated in the AVOD trial and those that did not.
- Statistical analysis using Pearson's chi-squared test.
Main Results:
- Significant differences in AUD management were observed between AVOD and non-AVOD hospitals.
- AVOD hospitals used antibiotics less frequently (7% vs. 38%) and managed fewer patients as inpatients (18% vs. 38%) compared to non-AVOD hospitals.
- Patient characteristics were comparable across all participating hospitals.
Conclusions:
- While most AUD patients were managed according to guidelines, significant inter-hospital variation exists.
- Previous participation in clinical studies may enhance knowledge and adherence to AUD management guidelines.
- Clinical trial experience appears to correlate with more conservative AUD management strategies.
