Role of Ephrin-Eph Signalling in Intestinal Stem Cell Renewal

Erythropoietin-producing hepatocellular carcinoma receptor (Eph) and its ligand, Eph receptor-interacting protein (Ephrin) were first discovered in the human carcinoma cell line, hence the name. Ephrin-Eph interaction guides cells to reach their appropriate location in adult tissues. They also play an essential role in the immune system by helping in immune cell migration, adhesion, and activation. Based on their structure and function, Eph is divided into two classes — EphA and EphB.

Summary

Systemic mastocytosis (SM), a rare hematological neoplasm, can cause gastrointestinal dysfunction. Prompt diagnosis and treatment with antihistamines can resolve SM-related gastrointestinal symptoms.

Background:

Eosinophilic gastrointestinal disorders (EGIDs) cause gastrointestinal dysfunction due to eosinophil accumulation.
EGIDs are often diagnoses of exclusion, requiring consideration of various differential diagnoses.
A history of anaphylaxis suggests systemic mastocytosis (SM) as a primary differential.

Observation:

Systemic mastocytosis is a hematological neoplasm involving clonal mast cell accumulation in systemic tissues.
Gastrointestinal manifestations are a key feature of systemic mastocytosis.
Diagnosis is confirmed by elevated serum tryptase and positive c-kit/CD117 staining.

Findings:

Gastrointestinal symptoms associated with systemic mastocytosis can be effectively managed.
Combined oral antihistamines provide rapid resolution of gastrointestinal manifestations in SM.
Identifying SM is crucial for appropriate patient management and symptom relief.

Implications:

Recognizing systemic mastocytosis is vital for patients presenting with eosinophilic gastrointestinal symptoms and a history of anaphylaxis.
Prompt diagnosis and treatment can significantly improve patient outcomes and quality of life.
This highlights the importance of considering hematological differentials in complex gastrointestinal disorders.
