Abstract:
Male, widower, smoker of 20 cigarettes / day, 75 years old, who lives alone, goes to the pharmacy to pick up medication. We observe an excess of boxes of metformin in electronic prescription, since it always says "that you still have at home." It is a patient selected by his reference hospital to be included in the "Telémaco" project, since he uses hospital dispensing medication. We provide Pharmaceutical Care with a multidisciplinary and multidimensional approach, according to the Capacity-Motivation-Opportunity methodology, which allows us to work in collaboration with the Hospital Pharmacy Service for common objectives, after patient stratification. The final result is the control of their health problems, working together to achieve the same goal, facilitating care support and those social resources that best adapt to the needs of the patient, to achieve the optimal state of well-being and quality of life of the patient.This model of Pharmaceutical Care represents a change in the way we work and record pharmaceutical interventions as we knew it until now at the level of community pharmacies and hospitals. Dual Pharmaceutical Care is provided, since it arises from the combination of Face-to face Care with Digital Care, thanks to the support of new technologies to be closer to professionals, accompanying patients, increasing their knowledge and optimising response time, in coordination with their care team. It involves optimising resources, devoting more time and effort to those patients who need it most.We work by objectives from the interview with the patient, generating interventions oriented to the specific characteristics of each patient.It is a model of continuous, not episodic Pharmaceutical Care, thanks to the use of new technologies, to improve health outcomes. This Pharmaceutical Care model generates satisfied patients and professionals.
