Area of Science:

  • Interventional Radiology
  • Medical Imaging Analysis
  • Vascular Anatomy

Background:

  • Prostatic artery embolization (PAE) is a treatment for benign prostatic hyperplasia.
  • Accurate identification of prostatic arteries is crucial for successful PAE.
  • Existing methods for vessel identification can be time-consuming.

Purpose of the Study:

  • To evaluate the performance of Vessel Tracking software (EmboGuide) for identifying prostatic arteries during PAE.
  • To assess the software's sensitivity and inter-reader agreement in determining feeding arteries using Conebeam-CT (CBCT).

Main Methods:

  • Retrospective analysis of 120 intraprocedural CBCT scans from 60 patients undergoing PAE.
  • Software (EmboGuide) used for automatic feeder detection by three experienced interventional radiologists.
  • Comparison of software-identified arteries against a reference standard (DSA and expert segmentation).

Main Results:

  • The software achieved a sensitivity of 0.968 (90% CI).
  • Overall agreement among readers was 0.801 (90% CI).
  • Both sensitivity and agreement met predefined study specifications.

Conclusions:

  • EmboGuide software effectively identifies prostatic arteries for PAE using CBCT.
  • The findings support expanding EmboGuide's intended use to include PAE procedures.
  • This technology may enhance the efficiency and accuracy of PAE.
