Background:

Purpose of the Study:

  • To evaluate the preventive effect of switching GBCAs on the recurrence of GBCA-associated acute ADRs.
  • To identify factors influencing GBCA-associated acute ADR recurrence.

Main Methods:

  • Retrospective, observational, single-center study (January 2016 - July 2023).
  • Analysis of 238,743 GBCA-enhanced MRI examinations.
  • Comparison of ADR recurrence rates between patients who switched GBCAs and those who did not, using multivariable logistic regression and propensity score matching.

  • Acute ADRs occurred in 0.44% of GBCA-enhanced MRI examinations.
  • Among 373 patients with prior acute ADRs, 31.9% experienced recurrence.
  • ADR recurrence was significantly lower in the GBCA change group (OR: 0.35; P=0.03) and for allergic-like hypersensitivity reactions (aOR: 0.20; P<0.01).

Conclusions:

  • Changing GBCAs is associated with a reduced risk of GBCA-associated acute ADR recurrence.
  • This strategy may enhance patient safety in GBCA-enhanced MRI.
  • Further research could explore specific GBCA types and premedication protocols.
