Background:

  • Osteoporosis incidence and fracture risk rise with age, posing significant public health challenges.
  • Postmenopausal osteoporosis is characterized by increased bone resorption linked to declining estrogen levels.
  • The endocannabinoid system, particularly cannabidiol (CBD), shows promise in modulating bone formation and resorption.

Purpose of the Study:

  • To investigate the therapeutic effects of cannabidiol (CBD) administration in a rat model of post-ovariectomy-induced osteopenia (OVX).
  • To assess CBD's impact on bone quality, microarchitecture, and mechanical properties in osteopenic bone.
  • To evaluate changes in bone cell populations (osteoblasts and osteoclasts) following CBD treatment.

Main Methods:

  • Forty-eight female Sprague-Dawley rats underwent ovariectomy (OVX) or sham surgery and were divided into four groups.
  • CBD or vehicle was administered intraperitoneally for three weeks.
  • Bone mineral density (BMD), microarchitecture (micro-CT), mechanical properties, and cellular markers were analyzed post-euthanasia.

Main Results:

  • CBD treatment significantly improved lumbar vertebrae BMD and increased bone volume fraction (BV/TV%) and trabecular number (Tb.N) in the femoral neck.
  • Enhanced mechanical properties, including maximum force and stiffness, were observed in femurs and vertebrae of CBD-treated OVX rats.
  • CBD administration led to increased osteoblast numbers and decreased osteoclast numbers in osteopenic bone, without significantly altering RANKL and OPG expression.

Conclusions:

  • Cannabidiol (CBD) demonstrates therapeutic potential in improving bone quality and mechanical strength in an ovariectomy-induced osteopenia model.
  • CBD treatment positively modulated bone remodeling by increasing osteoblasts and decreasing osteoclasts in osteopenic bone.
  • Further investigation is crucial to establish the optimal timing and long-term efficacy and safety of CBD for treating postmenopausal osteoporosis.
