Summary

Primary ovarian leiomyosarcoma is an extremely rare cancer. This case highlights a patient with recurrent disease despite chemotherapy, emphasizing the need for greater awareness.

Background:

Primary ovarian sarcoma is exceptionally rare.
Primary ovarian leiomyosarcoma is even rarer due to the anatomical absence of smooth muscle in ovaries.

Observation:

A woman in her late 50s presented with a 6-month history of abdominal pain.
Pelvic mass identified via imaging, confirmed as ovarian leiomyosarcoma post-surgery.

Findings:

The patient received 6 months of gemcitabine and docetaxel chemotherapy post-operatively.
Recurrence of the ovarian leiomyosarcoma was detected in the pelvic cavity 8 months after surgery.

Implications:

This case underscores the rarity and aggressive nature of primary ovarian leiomyosarcoma.
Increased awareness is crucial for timely diagnosis and management of this uncommon malignancy.
