Summary

This study evaluated prophylactic para-aortic lymph node dissection in colorectal cancer patients. While complications were low, its value in predicting disease behavior and guiding treatment requires further investigation.

Background:

  • Colorectal cancer is a leading cause of cancer deaths globally, often metastasizing to the liver or lungs.
  • Para-aortic lymph node metastasis signifies advanced disease (Stage 4 AJCC, Stage 3 JSCCR) and occurs in approximately 1% of cases.
  • Neoadjuvant chemotherapy followed by para-aortic lymph node dissection is a current approach for metastatic para-aortic lymph nodes in colorectal cancer.

Purpose of the Study:

  • To assess the utility of prophylactic para-aortic lymph node dissection in colorectal cancer patients.
  • To evaluate the impact of this procedure on predicting disease behavior.
  • To identify patient selection criteria for prophylactic para-aortic lymph node dissection.

Main Methods:

  • A prospective study conducted from December 2020 to December 2023 at NCI, Cairo University.
  • Included patients with left colonic or recto-sigmoid cancer undergoing specific colectomies (left hemicolectomy, sigmoid colectomy, LAR).
  • All participants underwent formal mesenteric and prophylactic para-aortic lymph node dissection.

Main Results:

  • 60 patients underwent surgery with prophylactic para-aortic lymph node dissection.
  • The majority of cases involved descending colon (35%) and sigmoid colon (36.7%).
  • Postoperative complications included lymphorrhea (3.3%) and wound infection (10%). Recurrence was observed in 13.4% of patients.

Conclusions:

  • The study explored the role of prophylactic para-aortic lymph node dissection as an over-treatment protocol in colorectal cancer.
  • Findings aim to inform disease behavior prediction and patient selection for this procedure.
  • Further research is needed to fully elucidate the benefits and optimal application of prophylactic para-aortic lymph node dissection.
