Summary

This study introduces a new method using 4D Flow MRI to measure intracranial aneurysm stiffness. The aneurysm damping index (ADI) was significantly higher than normal, correlating with factors like beta-blocker use and smoking.

Background:

  • Arterial stiffness is a known biomarker for cardiovascular events and stroke.
  • Limited data exists on intracranial aneurysm stiffness.
  • 4D Flow MRI offers a potential tool for assessing this stiffness.

Purpose of the Study:

  • To assess the stiffness of intracranial aneurysms using 4D Flow MRI.
  • To introduce a novel method for quantifying arterial pulse wave damping through aneurysms.
  • To investigate clinical factors associated with aneurysm stiffness.

Main Methods:

  • 27 internal carotid artery aneurysms in 25 patients were analyzed.
  • 4D Flow MRI measured arterial pulse waveforms proximal and distal to the aneurysm.
  • Aneurysm Damping Index (ADI) was calculated by comparing waveform damping to the contralateral side.

Main Results:

  • The average ADI was significantly greater than 1.0 (1.18±0.28, P=0.0027).
  • Aneurysm ADI was significantly higher than the contralateral side (1.19±0.30 vs 1.05±0.17, P=0.029).
  • Multivariate analysis showed beta-blocker use (β=0.46, P=0.015) and smoking history (β=-0.22, P=0.024) correlated with ADI.

Conclusions:

  • A novel method using 4D Flow MRI quantifies arterial pulse wave damping in intracranial aneurysms.
  • The developed Aneurysm Damping Index (ADI) shows correlation with clinical factors like antihypertensive drug use and smoking.
  • Further research is needed to explore the clinical applications of evaluating aneurysm stiffness.
