This case report details a rare traumatic floating clavicle (ipsilateral sternoclavicular and acromioclavicular joint dislocation) successfully treated with surgical stabilization. The patient achieved excellent outcomes one year post-operation.

Background:
- Traumatic floating clavicle, or bipolar dislocation, is an uncommon injury involving both the sternoclavicular and acromioclavicular joints.
- This injury pattern presents unique challenges in diagnosis and management.
Observation:
- A 43-year-old male motorcyclist sustained a right shoulder injury with pain and limited motion.
- Radiographs and CT scans confirmed ipsilateral sternoclavicular and acromioclavicular joint dislocations with superior displacement of the AC joint and medial/lateral clavicle end injuries.
Findings:
- The patient underwent operative repair, stabilizing the sternoclavicular joint with FiberTape® suture and the acromioclavicular joint with a Dog Bone™ Button and suture cerclage.
- At one-year follow-up, the patient reported excellent outcomes with no complications.
Implications:
- Surgical intervention can provide excellent functional recovery for traumatic floating clavicle injuries in active individuals.
- This case highlights the importance of recognizing and surgically addressing combined sternoclavicular and acromioclavicular joint dislocations.
