Background:

Varicocele (VC) is a common cause of male infertility.
Predicting infertility risk in VC patients remains challenging.
Inflammatory markers may play a role in VC-associated infertility.

Purpose of the Study:

To develop and validate a nomogram predicting infertility risk in varicocele patients.
To identify key hemogram inflammatory markers associated with infertility in VC.
To provide a tool for early risk assessment and management.

Main Methods:

Retrospective analysis of 162 varicocele patients (2022-2024).
Division into infertile and fertile groups based on fertility status.
Logistic regression and nomogram construction using inflammatory markers (e.g., NLR, SIRI).
Validation using ROC, calibration, and decision curve analyses.

Main Results:

Significant differences in lymphocyte, monocyte, erythrocyte, RDW, MCV, MPV, PDW, NLR, PLR, MPVPR, and SIRI between groups (p < .05).
Erythrocyte count, PDW, NLR, and SIRI identified as independent predictors of infertility.
The nomogram achieved an AUC of 0.869 in the validation cohort, demonstrating good predictive performance.

Conclusions:

A novel nomogram effectively predicts infertility risk in varicocele patients using readily available inflammatory markers.
The nomogram offers a valuable tool for clinical assessment and management of male infertility in VC.
Further external validation is recommended to confirm the nomogram's generalizability.
