The Parathyroid Glands

The two pairs of parathyroid glands embedded within the posterior surface of the thyroid gland are restricted by a dense capsule around them. These glands comprise two distinct cell populations—parathyroid oxyphil and parathyroid principal cells- pivotal in calcium homeostasis.

Background:

Ectopic parathyroid adenomas are uncommon causes of primary hyperparathyroidism, often located outside the neck.
Their atypical locations, such as the mediastinum, complicate diagnosis with standard imaging.

Observation:

A 27-year-old female presented with severe hypercalcemia and elevated parathyroid hormone (PTH) levels.
Initial neck imaging was inconclusive; however, parathyroid scintigraphy identified a mediastinal ectopic adenoma.

Findings:

Robotically assisted thymectomy successfully removed the ectopic parathyroid adenoma.
Intraoperative PTH monitoring confirmed successful removal, leading to normalized calcium and PTH levels.

Implications:

This case highlights the critical role of parathyroid scintigraphy in diagnosing ectopic parathyroid adenomas.
A high index of suspicion and advanced imaging are crucial for managing hyperparathyroidism when conventional methods fail.
