Summary

The Zeiss Clarus 700 and Optos California cameras show strong performance in diabetic eye screening. The Clarus 700 demonstrated higher sensitivity and specificity for detecting diabetic retinopathy (DR) with staged mydriasis.

Background:

  • Diabetic retinopathy (DR) is a leading cause of blindness.
  • Effective screening programs are crucial for early detection and intervention.
  • Evaluating new imaging technologies is essential for improving screening efficiency.

Purpose of the Study:

  • To assess the performance of Zeiss Clarus 700 and Optos California cameras in a Diabetic Eye Screening Programme (DESP).
  • To compare the efficacy of these devices with and without staged mydriasis.
  • To determine their suitability for routine DR screening.

Main Methods:

  • 1497 patients (2993 eyes) from DESP and Virtual Eye clinics were included.
  • Non-mydriatic images from Clarus and Optos were compared to mydriatic reference standard photography.
  • Staged mydriasis was employed when non-mydriatic images were unassessable.

Main Results:

  • Zeiss Clarus 700 showed sensitivity of 94.2% for any retinopathy and 86.0% for referable DR.
  • Optos California showed sensitivity of 91.9% for any retinopathy and 77.6% for referable DR.
  • Unassessable rates were reduced with staged mydriasis for both devices.

Conclusions:

  • Both cameras are effective for DR screening, with Clarus 700 showing slightly better performance.
  • Wider fields of view detected more peripheral referable retinopathy.
  • A slight reduction in sensitivity for central macular lesions was noted with wider fields.
