Background:

  • Obstructive sleep apnea (OSA) is common, but polysomnography (PSG) has long waiting times.
  • Existing screening questionnaires lack accuracy in identifying severe OSA cases.
  • A need exists for efficient screening tools to prioritize patients for PSG.

Purpose of the Study:

  • To develop and validate a novel screening tool, the Obstructive Sleep Apnea Morphologic Scale (OSAMS).
  • To assess the utility of anthropometric and morphological features in predicting OSA severity.
  • To provide a practical alternative for identifying patients requiring urgent OSA diagnosis.

Main Methods:

  • Developed the OSAMS using 5 parameters: tonsil size, oropharyngeal passage, hypopharyngeal-laryngeal examination, neck circumference, and BMI.
  • Scored each parameter and correlated total scores with the apnea-hypopnea index (AHI) from PSG in 149 patients.
  • Evaluated OSAMS performance against existing OSA screening questionnaires.

Main Results:

  • A moderate positive correlation (rho = .491, P < .001) was observed between OSAMS scores and AHI.
  • An OSAMS score >4 demonstrated 80.3% sensitivity and 53.9% specificity for detecting severe OSA.
  • OSAMS outperformed existing questionnaires in screening for severe OSA within the study cohort.

Conclusions:

  • The OSAMS presents a viable alternative screening tool for severe obstructive sleep apnea.
  • Its basis in readily observable morphological characteristics makes it practical for physical examinations.
  • OSAMS can aid in prioritizing patients for polysomnography, improving resource allocation.
