Glottic Pyogenic Granuloma in the Setting of Helicobacter pylori Gastritis

Abstract:
Pyogenic granuloma is a benign vascular lesion that rarely involves the larynx. Known associated factors include trauma, hormonal changes, and laryngopharyngeal reflux, but no prior case has described glottic pyogenic granuloma in association with Helicobacter pylori gastritis. We report a 57-year-old man who presented with progressive dysphonia and was found to have a large hypervascular mass of the right anterior vocal fold causing partial glottic airway obstruction. Excisional biopsy confirmed pyogenic granuloma. Concurrent esophagoscopy with gastroesophageal junction biopsies revealed H. pylori gastritis confirmed by immunohistochemistry. The patient underwent surgical excision with complete resolution at two-week follow-up, and standard eradication therapy was initiated for the H. pylori gastritis. This case raises the possibility that H. pylori-associated gastritis may contribute to reflux-mediated laryngeal mucosal injury and granuloma formation and suggests that evaluation for H. pylori may be warranted in patients with laryngeal pyogenic granuloma.
