Background:

End-stage lung disease necessitates lung transplantation as a life-saving intervention.
Patient selection is critical, with various factors influencing transplant eligibility.
Identifying reasons for exclusion is vital for improving transplant outcomes.

Purpose of the Study:

To identify the conditions preventing lung transplant in patients with end-stage lung disease.
To analyze demographic data and reasons for transplant exclusion.
To evaluate the impact of performance status, weight, and smoking on transplant candidacy.

Main Methods:

Retrospective analysis of patients referred for lung transplant evaluation.
Inclusion of demographic data, diagnoses, and reasons for exclusion.
Utilized the Karnofsky Performance Status scale to assess functional capacity.

Main Results:

311 patients were evaluated; 106 (34%) were excluded from lung transplantation.
Leading exclusion reasons included poor performance status (50%), weight issues (46.2%), and active smoking (9.4%).
Idiopathic interstitial pneumonia and COPD were the most common underlying diagnoses.

Conclusions:

Impaired performance status is the primary barrier to lung transplantation.
Preventable factors like smoking and obesity significantly impact eligibility.
Enhanced pulmonary rehabilitation and multidisciplinary care are crucial for optimizing transplant candidates.
