Background:

An independent 73-year-old woman with a history of psychopathology presented with a forearm fracture after a fall.
Non-compliance with prescribed medications and suspected substance abuse complicated her care.

Observation:

Evaluation revealed excessive use of hypnotic-anxiolytic medications, potentially linked to the fracture.
Poor therapeutic adherence contributed to uncontrolled health issues and Negative Outcomes of Medication (NOM).

Findings:

Pharmacist consultation identified potential NOMs and suggested therapeutic alternatives for antihypertensive and antidepressant regimens.
Physician intervention involved medication adjustments: bupropion withdrawal, benzodiazepine deprescription, and NSAID replacement with analgesics.
The patient experienced improved cardiovascular health, enhanced quality of life, and increased autonomy.

Implications:

Effective multidisciplinary communication is crucial for managing cardiovascular disease and optimizing pharmacotherapy.
Collaborative healthcare approaches improve patient outcomes and resource utilization.
Addressing medication non-compliance and optimizing drug regimens are key to preventing NOMs in elderly patients.
