Background:

  • Brainstem cavernous malformations (BCMs) pose a significant risk of hemorrhage.
  • The efficacy of radiosurgery in preventing BCM hemorrhage is debated.

Purpose of the Study:

  • To evaluate the protective benefit of radiosurgery against hemorrhage in patients with BCMs.
  • To assess the impact of radiosurgery on neurological function and adverse effects.

Main Methods:

  • A prospective observational multicentre study involving 377 patients with BCMs.
  • Patients were divided into observation (standard care) and radiosurgery groups.
  • Multivariate Cox regression and propensity score matching were used for analysis.

Main Results:

  • Radiosurgery was associated with a significantly lower incidence of hemorrhage (10.3% vs. 25.0%).
  • Radiosurgery was an independent protective factor against hemorrhage (HR 0.379, P=0.004).
  • Fewer new or worsening neurological dysfunctions were observed in the radiosurgery group (16.5% vs. 28.9%).

Conclusions:

  • Radiosurgery appears to be associated with a reduced rate of hemorrhage in brainstem cavernous malformations.
  • The findings suggest radiosurgery may benefit selected patients, though further research is needed.
