Background:

  • Lower urinary tract (LUT) symptoms, particularly overactive bladder (OAB), affect over 80% of multiple sclerosis (MS) patients.
  • The link between MS-related white matter (WM) changes and OAB is not well understood.

Purpose of the Study:

  • To evaluate microstructural WM differences in MS patients with OAB compared to those without LUT symptoms and healthy controls using diffusion tensor imaging (DTI).
  • To assess the association between clinical OAB symptom scores and DTI-derived indices.

Main Methods:

  • Diffusion tensor imaging (DTI) was used to scan 29 female MS patients (17 with OAB, 9 without LUT symptoms) and 14 healthy controls (HCs).
  • Tract-based spatial statistics analyzed fractional anisotropy (FA) differences, and the Urinary Symptom Profile questionnaire measured OAB severity.

Main Results:

  • MS patients exhibited significantly reduced FA in WM compared to HCs (p=0.004).
  • A significant inverse correlation was found between FA in frontal and parietal WM and OAB scores (p=0.021).
  • MS patients with OAB showed trends towards lower FA in the frontal lobes and non-dominant hemisphere compared to MS patients without LUT symptoms (p=0.072).

Conclusions:

  • WM lesions in MS contribute to OAB symptoms, highlighting WM's role in LUT neural control.
  • DTI effectively identified associations between OAB severity and WM changes, enhancing the current LUT model.
  • Regional WM involvement, particularly in specific tracts and the non-dominant hemisphere, can lead to OAB in MS, correlating with anisotropy changes.
