Muscles of the Shoulder01:23

Muscles of the Shoulder

The muscles surrounding the shoulder girdle, including the clavicle and scapula, primarily stabilize the scapula. This stable base allows other muscles to move the humerus effectively. Scapular movements often mirror those of the humerus and extend its range of motion. For instance, raising the arm above the head would not be feasible without simultaneous upward rotation of the scapula.

    Area of Science:

    • Orthopedic Surgery
    • Sports Medicine
    • Biomechanical Engineering

    Background:

    • Recurrent anterior shoulder instability is a common clinical challenge.
    • Bankart repair and Latarjet procedure are established treatments.
    • Arthroscopic techniques are gaining prominence due to surgical advancements.

    Purpose of the Study:

    • To assess the need for a classification system for traumatic anterior recurrent instability.
    • To evaluate current surgical techniques, including arthroscopic approaches.
    • To introduce a novel classification system (BoTH) for treatment guidance.

    Main Methods:

    • Systematic review of relevant literature.
    • Analysis of 12 systematic reviews, 3 prospective studies, 7 non-randomized studies, and 1 biomechanical study.
    • Development of the novel Bone-Tendon-Humeral (BoTH) classification system.

    Main Results:

    • A wide array of surgical procedures exists for traumatic anterior recurrent instability.
    • Arthroscopic techniques are increasingly utilized.
    • The proposed BoTH classification system aims to streamline treatment selection.

    Conclusions:

    • Current evidence supports the development of a structured approach to managing anterior shoulder instability.
    • The BoTH classification system offers a simplified decision-making framework.
    • Further research may validate the clinical utility of the BoTH system in guiding surgical interventions.
