    Summary

    Involutional macular thinning predisposes eyes to macular holes. Pigment epithelial defects and lack of posterior vitreous detachment significantly increase this risk, as detailed in our study.

    Background:

    • Involutional macular thinning is a precursor to idiopathic macular holes.

    Purpose of the Study:

    • To identify risk factors for idiopathic macular hole development in patients with involutional macular thinning.
    • To propose a theory for the pathogenesis of idiopathic macular holes.

    • Longitudinal follow-up of 93 patients (102 eyes) with involutional macular thinning.
    • Ophthalmoscopy and fluorescein angiography were utilized.
    • Analysis of risk factors including posterior vitreous detachment and pigment epithelial window defects.
