Maneuvers activating upper limb tremor can help differentiate essential tremor (ET) head tremor from dystonic tremor (DT) in cervical dystonia. Specific arm positions altered head tremor in ET but not DT patients.

Background:

  • Head tremor presents diagnostic challenges, particularly as an isolated symptom.
  • Differentiating essential tremor (ET) from dystonic tremor (DT) in cervical dystonia is clinically significant.

Purpose of the Study:

  • To evaluate if upper limb postural tremor maneuvers can distinguish head tremor in ET versus DT.
  • To identify specific clinical or accelerometric features differentiating these tremor types.

Main Methods:

  • 48 patients with head tremor (25 ET, 23 DT) underwent clinical and accelerometric assessments.
  • Tremor evaluation included head and upper limb analysis during specific arm-position tasks.
  • Tasks were designed to activate postural tremor in the upper limbs.

Main Results:

  • Head tremor power and clinical scores did not differ significantly between ET and DT groups at baseline.
  • ET patients showed increased head tremor during outstretched and wing-beating arm positions, unlike DT patients.
  • Coherence between head and upper limb tremor was consistent; tremor stability index showed no significant differences.

Conclusions:

  • Task-induced modifications in head tremor may serve as a diagnostic aid for differentiating ET from DT.
  • Further research is warranted to refine diagnostic strategies for head tremor.
