Elevated adolescent diastolic blood pressure (DBP) is linked to increased stroke risk in young adults. Even slightly elevated DBP (70-79 mmHg) in females raises stroke risk, highlighting the need for sex-specific BP guidelines.

- Adolescent diastolic blood pressure ≥80 mmHg is a significant risk factor for young adult stroke in both sexes.
- Elevated systolic BP in adolescence does not appear to be a stroke risk factor.
- Findings suggest the need for sex-specific BP monitoring and risk assessment in adolescents.

Monitoring BP in both arms during the initial assessment is advisable, as the systolic value may differ by five to ten mm Hg between arms. For subsequent BP assessments, use the arm with the higher reading.
