Coronary Artery Disease-Reporting and Data System (CAD-RADS) calcification may overestimate stenosis. CTCA-derived Fractional Flow Reserve (FFRCT) showed no significant difference between calcified and non-calcified stenoses across most CAD-RADS categories, except for CAD-RADS 3.

Conclusions:
- Coronary calcification does not consistently lead to an overestimation of functional stenosis severity by FFRCT across all CAD-RADS categories.
- CAD-RADS 3 with calcification may suggest functional underestimation by FFRCT, though not confirmed by ICA.
- FFRCT provides valuable functional information, but the influence of calcification needs consideration, particularly in intermediate CAD-RADS categories.
