    Summary

    Spontaneous enophthalmos can occur due to maxillary sinus fibrotic contraction, not just bone demineralization. Releasing this tissue corrected the condition without implants.

    Observation:

    • A case of spontaneous enophthalmos in a 24-year-old female is presented.
    • This patient did not exhibit demineralization of the orbital floor.
    • The enophthalmos was attributed to fibrotic contracture within the maxillary sinus pulling the orbital floor downward.

    Findings:

    • Surgical release of fibrotic tissue via a combined orbital and sinus approach successfully corrected the enophthalmos.
    • No alloplastic implants were required for the correction.
    • This case demonstrates that fibrotic contraction, not solely bony destruction, can cause spontaneous enophthalmos.

    Implications:

    • Fibrotic contracture of the maxillary sinus is a potential cause of spontaneous enophthalmos.
    • A combined orbital and sinus surgical approach can effectively treat this condition.
    • This finding expands the differential diagnosis for spontaneous enophthalmos and highlights a less invasive treatment option.
