Background:

Purpose of the Study:

  • To assess the effectiveness and safety of mini-ECIRS for complex renal and ureteral stones.
  • To analyze postoperative complications associated with mini-ECIRS.
  • To correlate stone burden and complexity (using Guy's Stone Score) with outcomes.

Main Methods:

  • Prospective analysis of 60 consecutive patients undergoing mini-ECIRS.
  • Patients categorized by Guy's Stone Score (GSS).
  • Stone-free rate (SFR) assessed via CT scan 4 weeks post-procedure (Grade A: no stones, Grade B: <2mm, Grade C: 2.1-4mm).

Main Results:

  • High stone-free rate achieved: 80% Grade A (no stones), 20% Grade C (<4mm fragments).
  • Overall complication rate was 21.7% (Clavien-Dindo Grade I-II).
  • Patients with lower GSS (1-3) demonstrated significantly fewer complications and higher SFR compared to higher GSS.

Conclusions:

  • Mini-ECIRS is a safe and effective treatment for complex renal and ureteral stones.
  • Higher GSS (4) is associated with increased need for secondary procedures and higher complication rates.
  • Further randomized studies are warranted to validate these findings.
