Background:

  • Poststroke fatigue impacts approximately 50% of stroke survivors, leading to substantial personal, societal, and economic burdens.
  • The Fatigue After Stroke Educational Recovery (FASTER) study investigated a group-based educational intervention for poststroke fatigue.

Purpose of the Study:

  • To assess the effectiveness of a fatigue management group (FMG) intervention compared to general stroke education for reducing poststroke fatigue.
  • To evaluate outcomes at 6 weeks and 3 months post-intervention.

Main Methods:

  • Two hundred patients with significant poststroke fatigue were randomized into either a control (general stroke education) or intervention (FMG) group.
  • The FMG intervention consisted of weekly psychoeducation sessions over 6 weeks.
  • Coprimary outcomes included the Fatigue Severity Scale (FSS) and Multidimensional Fatigue Inventory-20 (MFI-20) total scores.

Main Results:

  • No significant differences were found between the FMG and control groups in FSS or MFI-20 scores at 6 weeks or 3 months.
  • Adjusted mean FSS scores at 6 weeks were nearly identical (mean difference -0.13, P=0.92).
  • Secondary outcomes, including sleep, pain, mood, and quality of life, were also similar between groups.

Conclusions:

  • The fatigue management group intervention did not demonstrate significant group-level benefits over general stroke education.
  • Further refinement and investigation of educational group-based interventions for poststroke fatigue are warranted, considering individual-level impacts.
