    Background:

    • Splenectomy, the surgical removal of the spleen, is a common procedure for various hematological conditions.
    • Patients undergoing splenectomy face an increased risk of overwhelming postsplenectomy infection (OPSI).
    • Hereditary spherocytosis is a genetic red blood cell disorder often treated with splenectomy.

    Observation:

    • Three new cases of postsplenectomy sepsis are presented, occurring 14, 25, and 26 years post-splenectomy in patients with hereditary spherocytosis.
    • Two of the three new cases were fatal.
    • The literature review identified 25 cases of late-onset OPSI (≥10 years post-splenectomy), with 14 fatalities.

    Findings:

    • The mean age of onset for late-onset OPSI in this cohort was 37 years.
    • Pneumococcal infections were the causative agent in 76% (19 out of 25) of the late-onset cases.
    • Major predisposing conditions included trauma, hereditary spherocytosis, and idiopathic thrombocytopenia.

    Implications:

    • Patients with hereditary spherocytosis require lifelong vigilance for postsplenectomy sepsis, even many years after splenectomy.
    • Pneumococcal vaccination and prompt antibiotic treatment are crucial for managing OPSI risk.
    • Understanding long-term risks associated with splenectomy is vital for patient management and public health strategies.
