The lateral view of the cranium is dominated by temporal, sphenoid, and ethmoid bones.

Background:

Biparietal osteodystrophy (BPT), or biparietal thinning, is a rare condition characterized by symmetrical thinning of the parietal bones.
It predominantly affects women in their sixth and seventh decades of life.

Observation:

A 73-year-old female with osteoporosis and osteoarthritis presented with a vertex scalp mass, dizziness, and blurred vision.
The skull abnormality preceded the neurological symptoms and was noted after a history of head trauma.

Findings:

Laboratory tests were normal except for elevated vitamin D levels.
CT and MRI scans confirmed bilateral parietal bone loss without any cerebral abnormalities.

Implications:

This case underscores the need to consider biparietal osteodystrophy in the differential diagnosis of scalp abnormalities.
Dermatologists should be aware of BPT and its association with systemic conditions like osteoporosis.
