Angiographically occult arteriovenous malformations (AVMs) are rare brain vascular lesions. Early diagnosis via imaging and surgical removal are crucial for effective treatment.

    Area of Science:

    • Neurology
    • Neurosurgery
    • Radiology

    Background:

    • Arteriovenous malformations (AVMs) are typically visible on angiography.
    • Angiographically occult AVMs, especially without hemorrhage, are considered rare.

    Observation:

    • Three cases of histologically confirmed, angiographically occult brain AVMs are presented.
    • Patients presented with seizures or dizziness, diagnosed via computed tomography (CT) and radionuclide scans.
    • Preoperative diagnoses included granuloma and meningioma, with AVMs confirmed post-biopsy.

    Findings:

    • Histological confirmation of AVMs in all three cases despite initial misdiagnoses.
    • Literature review of 47 cases of angiographically occult AVMs is included.
    • Discussion of potential etiologies for these occult AVMs.

    Related Experiment Videos

    Implications:

    • Computed tomography (CT) scanning is vital for detecting these lesions.
    • Timely surgical intervention using operating microscopes is necessary for diagnosis and treatment.
    • Increased awareness and diagnostic vigilance for angiographically occult AVMs are warranted.
