A patient developed photosensitivity to ultraviolet A (UV-A) radiation while taking multiple medications. Discontinuation of quinidine and warfarin resolved the photosensitivity, suggesting a drug-induced reaction.

    Area of Science:

    • Dermatology
    • Photobiology
    • Clinical Pharmacology

    Background:

    • A 55-year-old female patient developed a photosensitive dermatitis affecting sun-exposed skin areas.
    • The patient was concurrently taking quinidine gluconate, warfarin sodium, furosemide, spironolactone, and digoxin following cardiac surgery.

    Observation:

    • Phototesting revealed a normal response to ultraviolet B (UV-B) radiation (290-320 nm).
    • The patient exhibited an exaggerated erythematous response to ultraviolet A (UV-A) radiation (320-400 nm) at a significantly lower dose (6 J/cm²) than typically required.

    Findings:

    • Discontinuation of quinidine and warfarin led to the complete resolution of UV-A hypersensitivity.
    • Resumption of quinidine alone, without warfarin, re-induced UV-A sensitivity.
    • Topical or intradermal administration of the drug preparations did not elicit reactivity, with or without UV-A exposure.

    Related Experiment Videos

    Implications:

    • This case strongly suggests that quinidine is the causative agent for the drug-induced photosensitivity.
    • The findings highlight the importance of considering medication history in patients presenting with photosensitive dermatitis.
    • Further investigation into the phototoxic mechanisms of quinidine may be warranted.
