Pyoderma gangrenosum (PG) can mimic squamous cell carcinoma (SCC) due to pseudoepitheliomatous hyperplasia (PEH). This case highlights the diagnostic challenge and importance of correlating clinical and histopathologic findings for accurate PG diagnosis.
