Background:

  • Minimally invasive aortic valve replacement (AVR) is a common procedure for severe aortic stenosis.
  • The Inspiris aortic valve and Cor-Knot device are utilized for AVR, particularly in complex cases like bicuspid valves.
  • Proper device deployment is critical for long-term prosthetic valve function.

Observation:

  • A 38-year-old woman with a type 0 bicuspid valve underwent AVR with a 21-mm Inspiris valve and Cor-Knot.
  • Postoperative echocardiography was initially normal, but four months later, moderate to severe aortic regurgitation was detected.
  • Prosthetic valve dysfunction was suspected, necessitating reoperation.

Findings:

  • Reoperation revealed perforation of the bioprosthetic valve at the noncoronary cusp.
  • The perforation was attributed to prolonged exposure to the Cor-Knot device.
  • The narrow deployment space likely contributed to the adverse interaction.

Implications:

  • This case highlights a potential complication associated with the Cor-Knot device during AVR.
  • Maintaining perpendicular apposition between the sewing ring and Cor-Knot tips is essential, especially in confined spaces.
