Preoperative smoking cessation interventions significantly increase the odds of patients quitting smoking before surgery. However, these interventions did not reduce the rate of postoperative complications in the reviewed studies.
Area of Science:
- Public Health
- Surgical Outcomes Research
- Addiction Medicine
Background:
- Smoking is a leading cause of preventable death and increases postoperative complication risks.
- The preoperative period offers a window for smoking cessation interventions.
- Systematic review and meta-analysis to evaluate preoperative smoking cessation effectiveness.
Purpose of the Study:
- To assess the effectiveness of preoperative smoking cessation interventions on smoking cessation rates.
- To determine the impact of these interventions on postoperative complications.
Main Methods:
- Systematic literature search for relevant studies.
- Random-effects meta-analysis of 14 studies (2940 participants).
- Primary outcome: smoking cessation; Secondary outcome: postoperative complications.
Main Results:
- Interventions significantly increased odds of quitting smoking by surgery (RR 1.64) and long-term follow-up (RR 1.38).
- No significant difference in postoperative complication rates between intervention and control groups (RR 0.81).
- Moderate quality of evidence due to high risk of bias and heterogeneity.
Conclusions:
- Preoperative smoking cessation interventions are effective in helping patients quit smoking.
- Current evidence does not demonstrate a reduction in postoperative complications.
- Recommendations include standardized outcome measures and research into patient perspectives.
