    Summary

    This study shows that acellular dermal matrix (ADM) is effective in two-stage breast reconstruction, with low complication rates. Using ADM can be beneficial and cost-effective for patients.

    Background:

    • Acellular dermal matrix (ADM) is used in breast reconstruction, but its effectiveness varies.
    • Opinions on ADM use in reconstructive surgery are divided due to differing results.

    Purpose of the Study:

    • To assess the long-term impact and effectiveness of a specialized acellular dermal matrix (ADM).
    • To evaluate ADM in a delayed two-stage breast reconstruction process.

    Main Methods:

    • Retrospective study of 46 patients (49 breasts) undergoing delayed two-stage breast reconstruction with ADM.
    • Analysis of postoperative complications, patient history, comorbidities, and adjuvant therapy.

    Main Results:

    • Low complication rates observed: 4 capsular contractures, 1 infection/dehiscence, 3 seromas.
    • Complications were more frequent in patients with post-radiation changes, comorbidities, and lower BMI.
    • Smokers did not show increased complications, except for wound dehiscence without expander exposure.

    Conclusions:

    • Acellular dermal matrix (ADM) prepared by CTB is beneficial in delayed two-stage breast reconstruction.
    • ADM can serve as an effective adjunct to prosthetic breast reconstruction.
    • The use of ADM in this context may also lead to reduced healthcare costs.
