Summary

Pamidronate effectively treated severe osteonecrosis (ON) in four patients, improving symptoms and stabilizing lesions within one year. This bisphosphonate treatment showed no significant side effects, suggesting it is a viable option for ON management.

Background:

  • Osteonecrosis (ON) is bone death due to compromised blood supply, with uncertain optimal management strategies.
  • High-dose glucocorticoid therapy is a known risk factor for developing osteonecrosis.
  • Severe ON presents with significant symptoms impacting patient self-care and quality of life.

Observation:

  • This study evaluated pamidronate treatment outcomes in four female patients with severe, multifocal lower limb osteonecrosis.
  • All patients had a history of high-dose prednisolone treatment and presented with CTCAE grade 3 symptoms.
  • Magnetic resonance imaging (MRI) and bone mineral density (BMD) were assessed before and one year after pamidronate treatment.

Findings:

  • Pamidronate treatment led to stable osteonecrosis lesions on MRI after one year.
  • Patients' clinical conditions improved significantly, resolving to CTCAE grade 1 (asymptomatic).
  • One patient experienced increased BMD (SD score of 2.9), with no other observed bisphosphonate-related side effects.

Implications:

  • Pamidronate may be an effective therapeutic option for improving functional impairment and pain in patients with severe osteonecrosis.
  • This study supports the use of bisphosphonates in managing glucocorticoid-induced osteonecrosis.
  • Further research is warranted to confirm these findings in larger patient cohorts.
