  • A 77-year-old male presented with persistent abdominal pain and fever post-ERCP.
  • CT scans revealed focal liver lesions, initially treated as a liver abscess without improvement.

Findings:

  • Liver biopsy demonstrated caseous granulomas.
  • Tuberculosis polymerase chain reaction (TB-PCR) confirmed Mycobacterium tuberculosis infection.
  • The patient was diagnosed with primary hepatic tuberculosis with subsequent dissemination.

  • Persistent symptoms and cholestatic liver function tests unresponsive to antibiotics warrant investigation for TB.
